N2 - Rationale. Endometrial cancer associated with uterine anomalies is extremely rare and is characterized by high diagnostic complexity. A comprehensive assessment of uterine cavities is crucial for gynecological practice, as it identifies the extent of the lesion and the appropriate treatment strategy. Case report. A case study describes of a 68-year-old female patient with complete duplication of the uterus and incomplete duplication of the vagina, who was diagnosed with synchronous endometrial cancer of uterine hemicavities (FIGO IB and IIIA). Preoperative imaging (MRI, ultrasound) made it possible to diagnose a tumor only in the right hemi-uterus, while involvement of the left hemi-cavity was only confirmed by postoperative morphological examination. A total hysterectomy of both uterine hemi-cavities and adnexa was performed followed by adjuvant radiation therapy. Conclusion. The obtained results highlight the need for separate diagnostic curettage of uterine hemi-cavities in cases of complete uterine duplication and demonstrates the feasibility of using a comprehensive preoperative examination algorithm (3D ultrasound, MRI, hysteroscopy) to optimize oncological outcomes.
